| For incoming Freshman and Juniors who are participating in high school sports next year and did not receive a physical last year.
Schedule:
May 21 - St. Maries Middle School, during school hours
May 22 - Kootenai High School (for middle and high school students) physicals available until 5 p.m.
May 23 - St. Maries High School, during school hours
The student will need to have a physical exam form completed and signed by parent or guirdian. Forms are available at the schools or at St. Maries Family Medicine clinic.
Please wear t-shirt and shorts that day, if school permits.
Sports physicals are conducted free by BCH/SMFM providers. |
