As a patient you are entitled to:
- Receive considerate and respectful care, and be assured of personal privacy and confidentiality;
- Receive care in a safe setting without abuse or harassment;
- Be assured of confidentiality of your medical records, as well as your ability to access information;
- Receive information to give consent prior for treatment, except in life threatening situations, which may not permit such an exchange.
Patient Responsibilities
As a patient, your responsibilities are to:
- Provide accurate and complete information about all matters relating to your health;
- Ask questions and seek information to help understand your illness and care;
- Get and carefully consider all information to give informed consent for procedures or treatment;
- Understand and weigh the potential consequences of refusals of care;
- Fulfill your financial obligations;
- Tell us opinions, concerns or complaints in a constructive manner;
- Tell the nursing staff about your comfort and pain.
It is important that physicians and other caregivers understand your wishes and personal values about medical treatment. BCH supports and complies fully with the law by asking each patient or their guardian if they have an advance directive. The nursing staff will ask for a copy to be placed in your medical record. Ask our Patient Financial Services Counselor to provide you with a form.
Your hospital stay and discharge
Upon arrival you will sign consent forms allowing staff to provide services and bill your insurance company. You may also be asked to sign additional consent forms for procedures such as surgery. Ask any questions you may have prior to signing any consent form. We are committed to sharing information to help you make appropriate choices. |  |
